Miscarriage. A pregnancy that stops developing. Abortion. Three events, very different in circumstance — but in systemic work they share one thing in common: they're almost never spoken about out loud. No funeral, no name, no place in the family album. As if nothing happened. But in the family system — it did.

This isn't only the language of constellations. Psychologists have a distinct concept for it — "disenfranchised grief", introduced by psychologist Kenneth Doka back in 1989: grief that society doesn't recognise as "significant enough" to be openly mourned. Miscarriage is one of the clearest examples — research indicates it occurs in 10–20% of known pregnancies, and yet it remains surrounded by near-total silence.

What happens when that place isn't acknowledged? Research shows that grief after pregnancy loss is comparable in intensity to other major bereavements and can include depression, anxiety and post-traumatic stress symptoms — sometimes lasting months or even years. Yet a woman is often not given even the space to grieve: "you'll have another one", "it was so early", "why be upset over something that barely existed". As a result, grief that was never lived through doesn't simply disappear — it settles: in the body, in an anxiety that's hard to explain, especially around later pregnancies.

The effect on the couple is just as significant. Research notes that partners often experience the loss very differently — and this mismatch in pace and style of grieving can itself become a source of tension. It's worth saying the reverse too: the same body of research (including a study published in Family Relations) found that some couples grow closer after a loss — when both partners turn toward each other rather than away. In my practice, when a couple's history includes several such unprocessed losses without that kind of turning-toward, it often becomes one of the hidden reasons their relationship cools over the years.

It also affects children born later. Sometimes they unconsciously sense that "someone is missing" — and take on a strange responsibility they can't quite explain even to themselves: to be "for two", to make up for a life that didn't get to happen, to live up to hopes that were never originally theirs. The order of places in the system shifts too: if the unacknowledged child was the first, the next-born unconsciously takes an earlier place that isn't really theirs — which can show up as a sense of "too much responsibility, too early".

What can help? Letting the place be acknowledged — simply, without drama: the child had their own time, their own place in the lineage, and they're remembered. Sometimes that's a simple ritual within the family; sometimes it's constellation work, where the place is symbolically returned to the system. And — importantly — giving grief its own time, without rushing it or dismissing it with "why be upset". And for partners: talking about it with each other out loud, rather than carrying it silently apart.
